lol...good question. maybe cuz that person seems like a good person because he/she is good looking but the ugly stranger seems not so good because he/she is ugly or unclean.

Human nature, natural selection,and self-preservation/protection all factor into one's behavior and attitudes.
But wait-a-hold-it: I'm not sure I agree with your premise about comfortably exchanging smiles with an appealing stranger...but maybe that's because I'm one of the ugly ones who's never been smiled at by strangers, so how would I know?

I guess we are all guilty of that one. I usually just smile back anyway, and continue about my business. Because the good looking stranger might be questionable, too. In this day and age you just never know. =)

I would believe that a good looking wouldn't need it as much, so to me it works most of the time in reverse.
Depending of the kind of smile, that's it.
And I smile back even more to Ladies and children...

I smile at everyone. Ingrained attitude from a Raggedy Ann doll I had once.. However, when the good looking guy smiles at me after I smile back I turn around to look and see who he was REALLY smiling at!

it relies upon on the position i become and what they gave the impact of for the most area i smile say hi yet when i'm in a foul community and they wearing gang shades or have a gun or some thing then i do no longer seem stare no longer some thing
