Treat with Jock itch spray or Athlete's foot spray (it's the same ingredient) along with Neosporin to clear up the redness and cracking of the skin. Change underwear daily or go without until the rash clears up. Wear loose fitting pants until the rash clears up.

Allergy to something in your detergent or fabric softener would be my first guess. Enzyme detergents like Gain are especially bad. Try switching to Ivory Snow or Ivory Flakes for a couple of weeks and stop using ANY form of fabric softener, including dryer sheets. (You don't need any kind of softener when you use Ivory, and softeners are hard on the skin.) Double rinse his underwear

He could have picked up crabs someplace - there are non-sexual ways of catching them. They are tiny lice that you can't normally see, and they cause intense itching. If that's it, he'll have to see the doctor to get a prescription for something to kill them, and you'll probably have to be treated as well.

If he shaves the area, have him let the hair grow - he shouldn't even trim. Shaving is very hard on the skin and encourages the growth of fungus, as lack of hair means lack of air circulation.

For temporary relief, get him some aloe vera gel - it's sold as a sunburn treatment but is also good for other skin problems.

I'm out of ideas, but I hope something in this list helps.

I call it crotch rot.. It's just bacteria that builds up because we don't get much air down there, and we sweat alot..You can tell him to try powder to keep it dry.. And if it pursist, then he's off to the Doctor..If he's scared, the doctor is only going to treat of take care only what he ask for.. doctors are cheap these days, and time is money, so you should not have any problem with going, by asking and telling them, that's all i want to see him/her for.

how bad is it? if it's plain itching, it may just be jock itch caused by the proliferation of fungi and bacteria in his scrotum. make sure he washes his genitalia thoroughly. if it's really bad, consult the doctor. it might be something serious...

It could be from clothes that are too tight; what laundry detergent is used.
I agree with you, better to check it out. If it isn't serious, that would be good. If serious, then treatment could be easier when caught in early stages.

it could be the underwear he's wearing. if he's wearing briefs he may want to switch to boxers. it also could be your laundry detergent. if it's jock itch, they have otc creams & sprays, try those. if they don't work see a doctor.
