I worked in a video poker place (also we are smokers) and we used a product called OroBan. The product is sold at Sam's Wholesale Clubs.
It is diluted with water in a spray bottle, 1/4 water 3/4 product.
You spray everywhere and it dries fairly quickly. I spray all my rugs, furniture, curtains and beds. I also use it in my washer. The laundry comes out softer and smelling very nice.

Febreeze pretty much should do it. You could put the couch outside for an airing for a while and there are a number of furniture shampoos available that should help.

Febreeze only makes it worse, it just mixes with the odor. I inherited a couch from chain smokers and had it proffessionally cleaned (not that expensive) and that helped alot.

In a car, you can throw some coffee beans down to absorb a smell. Try some coffee beans in the cushion.

Rent one of those rug doctors from the grocery store or walmart has them too. i think there about 20 bucks a day and it has an upholstery attachment... good luck

ask your local Vet, once I got some spray that eliminated skunk smell (also will rid smoke) there's also a product called odor assassin

Try shampooing it yourself, rent a machine. Otherwise have it professionally shampooed. Good luck!
