It just happens. I used to have it. But I realized I couldn't (and wouldn't want to) act that way around strangers, so I slowly stopped being so OC. And eventually my OCD just went away. But I don't know about other people. I think some seek the help of a psychologist.
The things I used to do were:
If I touch something (like a door knob) I had to count to a even number, before I could open the door.
And I would have to wash my hands everytime I touched anything.
And I had to organize my belongings a certain way or I couldn't (wouldn't) leave the house, go to sleep or do anything until I had done that.

A prominent feature of OCD is washing the hands or cleaning sth very much for a long time, out of control. This may come from the idea the sufferer has unconciously: done a 'dirty' work, feeling of guilt. So the person should be aware and get concious about it and be analysed by a psychologist or psychiatrist.

Any excessively repeated behaviours. Common ones are washing hands, flicking light switches and locking and unlocking doors. Some people need everything in even numbers, in lines... the list goes on!
There's many theories on what causes it. Your best bet is to pick up a good psychoparthology book, a general psych text book or go to google scholar and search OCD.

The brain. I have a mild case of OCD. Some examples are having to organize things exactly how you want them. You could spend up to a half hour doing just one item in a box. Also, people with OCD are afraid of germs. Have you ever seen the movie "Aviator" or "Match Stick Men"? Perfect examples of someone with a strong case of it.
