While towing or lifting a heavy load, the chain or rope snaps damaging properties & causing physical injuries. Yes, such accident could have been avoided but can anybody really pre-detemine the wear and tear of the chain or rope? Accident is used to differentiate untoward man-made incidents vs. purely fortuitous (god-made) events.
