I believe that the root cause of most, if not all, of the worlds problems can be traced to overpopulation. Global warming and all the ecological pressures are increased according to the number of people consuming the resources. Improved efficiency and reduced emissions will only work until the number of people increase to where the output and consumption levels are back where we started.
Social issues too stem from population pressure too. As resources and living space become depleted, people turn on each other and dream up political reasons to justify it. With space and plenty, people find contentment and fight less.
The pressures and demands on the resources and the contributions to the problems go up exponentially as the population goes up exponentially. But people ignore this problem and it is the one that fuels all the others.

Poverty and starvation. It is insane to me that there is so much of this in the world. These two factors are the cause of more death, wars, disease and unrest than anything else I can think of. Take a look at AIDS in Africa...there are so many organizations that strive to supply the necessary medicines and strive to find cures, yet people there live with it as though it was the common cold. Why? Because they are poor! They cannot afford the medications.....
The problem is so basic...there is not enough money to fight the disease. Prevention efforts in the western world centers around education, condom use and testing and counseling. Unfortunately, the lack of available health care dollars in the countries hardest hit has made medications scarce in those regions. from : http://aids.about.com/cs/aidsfactsheets/a/africa.htm
Every year 15 million children die of hunger
The Indian subcontinent has nearly half the world's hungry people. Africa and the rest of Asia together have approximately 40%, and the remaining hungry people are found in Latin America and other parts of the world. Hunger in Global Economy
One out of every eight children under the age of twelve in the U.S. goes to bed hungry every night. (site below)

The environment. If the worlds doesn't start taking the issue seriously soon, the economic growth it's so afraid to sacrifice will be a moot point, because the planet won't support our wasteful way of life anyway. Another closely related problem is world poverty.
The west needs to rethink not only the way it does things, but the very values that have lead us here.

First of all the direction America is going and wheter or not we will stop the dictator in the White House by raising our voices.Secondly, the entire free world is scared of terrorism. The best solution would be total unity in the free world. Which we can not even approach with Bush in office. Thirdly,Global Warming that is a true threat to our Mother Earth. And last but not least the horrors in Africa that should be addressed by concerned citizens of the world. Innocents drying up and dying in droves. That is my view. OOPs in America Health Care.

Religious freedom. TRUE religious freedom. Until TRUE religious freedom is attained the problems of the world will only get worse.
True religious freedom means that ALL things in ALL religions are sacrisanct and no one that is not a part of a particular religion has any authority to say or do anything with regard to how someone of a differnt religions, behaves, thinks, or feels. A persons religious belif muct be the sole and ONLY factor in decision making processes. Until THAT sort of religious freedom exists, NOTHING will get any better anywehre for anyone.

The issue that is of the most importance and concern to all the world at this time is "nuclear weapons" in the possession of third world countries that want to destroy any civilization that they cannot completely control.

probably nuclear weapons, although global warming may soon take over. the technical literature concerning the chances of nuclear war is frightening, a question of when rather than if is the general consensus. automated trigger systems mean that it will only take 1 decaying piece of russian infrastructure to ignite nuclear conflict. its also not unlikely that a terrorist group (chechens stealing one inside russia for example) could bring about the same scenario. none of it will matter if the earth continues to grow hotter and increased carbon omissions take us beyond the fast approaching "tipping point" where it will be impossible to prevent large scale devastation through flooding, crop failure, drought, scarcity of clean water, arable land etc. wars for those latter two commodities i list will make the current wars for oil look civilised.

It is not terrorism
It is not poverty
It is not health problems
It is the inability to tackle the problems from its very base and find solutions.
It is the inability to expressly condemn what is the root cause of each issue.
In brief......... ignorance and fear are the most important issues of the world at any point of time.
