Mail a letter to your ex-employer about your change of address and request the w-2's to be mailed to your new address. Also make sure you have a change of address filing with the USPS so all mail can be forwarded.

Keep a copy of the letter for your tax records.

Employers in the US have untill the last day of January to mail out w-2's.

If the company has one, you would need to notify the Human Resources or Payroll Dept. Send it Certified mail, as proof you sent the request as well as who received the letter.

Note: Generally, your last paystub will contain most, if not all, the info you need to complete your taxes.
