Don't be so concerned with numbers-- How you look is how you look; old or young, the number doesn't matter. If you feel you look older than others your age, try to see if you have some health issues you should work on-- Most premature aging stems from poor health.Or update your wardrobe a bit. If you feel you look fine--and you feel fine--- just laugh off your friend's amateurish attempt at humor.
On your friend's next birthday, bake a cake and put 15 extra candles on it--- or buy a funny card that has a big 60 on it if they are only 45--- Laugh! You'll look younger and feel better!
I always tell people I'm older than I am--- then they exclaim how GOOD I look! My friend has lied about her age for years--- but the years show! So people who don't know her well think she looks terrible for her age-- Go figure!
You are not your body--- your body will die, but you will not die. Be more centered in your real identity--- then people can't get you so riled up over little things.

Not a very good B-Day lunch. Not a very good friend as well. Friend should be reminded that these jokes are for April Fools Day. I think the friend wanted to be the centre of attention here instead of the birthday person.

She meant it as a joke, just for laughs. Especially since she told you about paying the waitress. It was just a joke. Don't take it too seriously. It was completely harmless.

Because your friend is mean and insecure. Apparently it made that person feel better to attempt to humiliate you. I would tell the person how you feel and if anything like that happens again, truly evaluate if this is a friendship or a former friend.

As a joke!!! If a friend took you to lunch and paid for it, I'm sure the friend wanted a few laughs--you're not offended by this are you? I'm sure that's not what the friend intended.
