Joaquin has a scar on his lip. According to Joaquin: "When my mon was pregnant with me she was in a hammock on the beach and seh got these intense pains, and she felt as if some force, God or whatever, had caused this. Then I`m born. A few weeks later she takes me to the doctor, he sees me and says, `Who did this harelip operation? That`s the best operation I`ve ever seen.` Mom said, `No one. He wasn`t born with a harelip.` I was just born with the scar."
The birth scar is a microform, almost taking the form of a cleft, as the tissues just reached far enough to join up; since the bone and muscle tissues joined correctly no surgery or cleft repair was required.

Contrary to popular belief, the scar on his lip is not the result of a repaired cleft lip or palate. It is simply a birth mark. Phoenix has stated in interviews that, while pregnant with him, his mother felt a sharp pain one day, and he was born with a mark on his lip.

According to IMDb website, he is quoted as saying it is a birthmark, not a scar or fixed cleft lip.

I heard somewhere that it was a cleft palate that he had surgery on when he was a baby.
