I think that is the one where Sawyer and Kate have sex, but anyway, "The Others" blackmail Jack into operating on Ben because he has a tumor on his spine.
During the surgery Jack knicked the kidney or liver I can't remember which, but he told "The Others" to get Kate on the walkie because there was only a small amount of time before he let Ben die.
He told Kate to run, but he has no idea that they are on a completely different island than the one they crashed on and it is a long swim between them.
That is all I can remember off the top of my head.
