HEMI is short for hemispherical, half of a sphere, which is the shape of the combustion chamber in early Hemi engines from Dodge/Mopar and now in modern day Dodge/Chrysler.

The concept behind Hemi is the shape of these chambers to produce maximum airflow. This coupled with 2 valves per cylinder produce high-output horsepower in Hemi engines.

Today through modern technology the Hemi engines have been engineered to produce maximum horsepower while offering substantial fuel economy.

If you mean what makes an engine a hemi, the most obvious design feature noticeable is the spark plugs go through the top of the head rather than the side.
The top of the piston is a domed shape (convex) not flat.
