an electric field of 640V/m is desired between two parallel plates 11.0mm apart. How large of a voltage should be applied? How do I arive at the answer?

The answer is that the field is calculated as the voltage between the plates divided by the distance so in your case you have

V / (.011meters) = 640 now solve for V It should be a smaller than 640 Volts as that is what you would need to get that field strength if the plates were 1 meter apart.
