Use a heating pad.
Take pain relievers with NSAIDs, like Aleve or Motrin
Mild exercise
Warm bath

Use any of these or in combination. What works varies from person to person.

You may want to schedule a visit with your gynecologist. He can determine what may be causing the extreme pain (mine was due to ovarian cysts). Right now, he has me on birth control pills, which should alleviate some of the pain (and heavy flow) that I am experiencing. The only over the counter remedy that has worked for me is to take four 200 mg advil (or other meds with ibuprofen) every 4 hours. This would be the equivalent to the heavy duty stuff your doctor would give you. The key is to start taking them AS SOON as you feel the cramps coming on and be consistent with the 4 hour rule. Don't wait until the pain is bad before you start the pills. You want them in your system so that when it does get bad, you won't even feel it. This has worked tremendously for me.

Motrin is the easiest way to get rid of cramps. If they are severe and you are sexually active, you should see a doctor or head to the clinic - you could have something that needs further medical testing. If you're young, severe cramps seem to be pretty common for teens - the older you get, the less they will hurt. Birth control will also alleviate severe cramps long term.

All of these suggestions are good. However, if you still experience severe pain you may want to consider birth control pills. I used to have very painful cramps, but now that I am on the pill I rarely have any type of pain or menstrual symptoms.

Take 1 or 2 Ibuprofen, depending on your size / weight, for 2 or 3 days before you start your cycle.

Midol, Tylenol, or Advil, and Heating pads. Heat seems to help best though, they have those self warming stick pads at wal-mart you just open the pack and stick them on your tummy and they heat up really warm. Cramps seem to go away.
