It doesn't fall off, but it also doesn't work very well. Most over-endowed men can't get erections, and there are a few well-known cases of men who passed out every time they got an erection because it took so much blood away from the other head.
But mostly what you see in porn is just fake. It's better not to watch this crap - you start to think the fantasy they portray is real life, but it is not anything like reality, and you will be let down bigtime if you expect reality to be like some fake porn.
