offer to let her babysit for you anytime she wishes, that would be all the present anyone could ever want

I'm 28 married with 2 kids and I think getting a gift certificate for a massage or facial&pedicure would be awesome. You could also offer to watch her kids while she goes.

But some people are not comfortable with masages and such. If that's the case then a gift certificate to somewhere nice, (outback steakhouse, red lobster) and you insisting on babysitting that night (maybe at your place so the kids can stay the night) would be really cool.

You have some great suggestions here. I would pick one of the suggestions below BUT I would also write her a heartfelt letter. Tell her thank you for her advice, the shower, and other things she has done for you. Go into detail in the letter listing specific things you loved advice wise ad shower wise. Write it in pretty paper. Your boyfriend has a very sweet sister. You are very lucky!!

Good luck with the new baby!! :)

I agree with naming the baby after her. That is a gift that will last a lifetime unlike getting her something or giving her a night out, since you will probably end up babysitting quite a few times for her in the future.

Gift certificate for a restaurant / night out? Plus an honest sincere thank you note written from the heart :)

A gift certificate for a day of special pampering for her. For a massage, or a manicure & pedicure. Take her out for lunch. Just make the gift or gesture about something she likes.

Give her a gift certificate to a store where she can buy some thing she has been wanting.
Since you are close to your delivery date it would not be safe for you to travel for more than an hour to baby sit for her.

Offer to watch her kids so she and her husband can go out together without the kids. I know thats one of my very favorite presents to get lol. If you don't feel that that is enough, or you just can't watch her kids you could get them a gift card to a resturaunt or a movie :) Hope this helps!

How about this... give her and her husband a gift certificate for dinner and/or a movie, then watch their kids one evening so they can have a date night together. They would probably enjoy that more than any physical thing you could give them!

The best gift doesn't cost a dime. The best gift for her would be to use the gift of wisdom that she has given to you as far as parenting. Also, be appreciative of everything that she has done for you even if you seem overwhelmed. Congrats to you!
