I have a really weird voice.
Most people can't understand me because I speak too low, but I'm shy, when I get confidence I tend to speak really fast and people can't understand me.
I change accent really easy.
And I have never been able to scream, as soon as I scream I start coughing really bad. What can I do?

I am a music major in voice. I know singing is different than speaking. But there are certain exercises you can do with your voice every day to help you shape your words, and your mouth. You should also see a speach therapist. Also, singing is a proven way to get rid of speech impediments, also reading, like poetry, out loud everyday that will help your speech.

Go to a speech therapist. The only way to improve something is to keep pracitcing at it. The same is for speaking. You might also have to see a medical doctor- there are medical disorders that can affect your voice. You may have one of them if you cannot scream. I'd go look into that right away.
