Depends on your definition.
Lots of movie villains and heroes, especially in sci-fi, kill thousands or even millions of people in one shot, such as Luke blowig up the Death Star, or the Death Star blowing up a whole planet, Alderran. That would make Peter Cushing as Grand Moff Tarken definitely in the top ten for ordering a whole planet blown up.
The Vogons in the Hitchhiker's Guide to the Galaxy blew up the Earth and everyone on it except 1 person.
Lots of action movies will have hundreds of people getting killed by bombs and such.
Going for personal, actually killing people one-at-a-time (even if it's with a machine gun) on-screen killing? That's tough to say.
Horror movies rarely have body counts over a dozen, but we're quite sure the victims are D-E-A-D, and sequels can multiply the count.
Probably a kung-fu actor is best for personal kills/body count, though we're not usually sure if the people are dead or just knocked out. Whatever the most famous running series of kung-fu movies is, the lead character is probably your winner.
I give a thumbs up to the person who mentioned Uma Thurman in Kill Bill. We're pretty sure the Japanese gang had 88 members and she killed/seriously maimed almost all of them.

I'm going to guess Jason from the Friday the 13th movies...he has a pretty high body count. LOL

I will definitely have to say Legolas from the lord of the rings. Granted the "orcs", "oliphants", and "Urikai" that he kills are not real but we are talking about fictional movies here so... yeah definitely Legolas.
