You need to choose the country/countries you wish to visit first. Most of the EU countries are also members of the Schenghen agreement (with the exception of Ireland, UK and the new member states).
So, make a route and apply for a visa in the Embassy of the first Schenghen member state you are going to visit. They /or their web page/ will tell you what the required documents are.

The Schengen country you wish to spend most of your time in is the country you apply to for a Schengen visa. If you are unsure, or spending your time equally, apply to the Schengen country you are going to visit first.

You can apply at the embassy in the UK. Contact details of the embassy can be found on the Foreign Ofice website http://www.fco.gov.uk/ under the About the FCO/Directory menu found on the left of the page.
