The First Man would be better then the First Gentleman to describe Penis Clinton, because he is not a gentleman. The First Lady-boy would also be all right, but it might be an insult for the lady-boys in Thailand.

Great question.... I would guess "gentleman"...since the phrase has always been... "Ladies and Gentlemen" (when announcing), so that very well may be the proper way to address him.
