It means the address in which you live, and then the town. ie street adress is : 3 Alison street

They are asking for the address where the Dr lives or works. If the paperwork is asking for your doctor's details then they mean his office address. Where you would visit him or where is office is.

A mailing address would be the address where mail is sent. A post office box can be used to receive mail while visitors to the doctor would go to the office.

If for some reason we are talking about where someone lives then the doctor's street address is their physical residence.

Included is everything related to the number of name of the building, the name of the street or lane, and any internal mail code if we are talking about a large complex with mail stations or other internal codes for the buildings, floors, etc.

The stuff between the person's name and the city that you would need to find them physically if they are in that day.

Town and city mean the same thing.

State is the province our county in the UK.

Zip would be the full post code recognized by the postal service for the country.

You live in a town. Within that town are places for cars to drive. These place for cars to drive are called streets (or roads or lanes or boulevards). Along these streets are buildings. These buildings have a numerical value associated with them. If you place this numerical value next to the name of the street, the result is the street address.

The Dr's street address is the identifying information required if you wanted to go out in the street and visit her/him (e.g. suite 176/first floor/ 1798 Forbes Street). In your example the town, state and zip are then required separately.

Street address is as opposed to postal address which might be for example PO Box 15435a.
