It someone didn't intend to do something wrong I forgive.
If someone intentionally hurts another person or an animal, and particularly when someone intentionally hurts any of my kids, I will never forgive. People say that forgiving is "divine", so I leave the forgiving up to God and choose, instead, to never forgive intentional harming. That doesn't mean I spend all my time thinking about what the person did. It just means that some actions are unforgivable and should never be forgiven.
When it comes to what is unintentional or what was an honest mistake, though - as long as someone acknowledges or apologizes I'm pretty forgiving.

I don't think anybody can say that they always forgive, because all too often we act emotionally rather than rationally when we are wronged.
However, I try always to forgive and understand people, simply because I realise that I don't know or understand exactly why they acted in the way they did and never will, simply because I don't live their lives and don't know their circumstances.
At the end of the day, not forgiving is a judgement against somebody, and judging people is wrong.

I forgive but it is hard to forget because it is a mistake we make as well, it takes two? And I only remember it so I won't do a repeat of the same mistake. But basically it is better to think about good things and future and not dwell in the past. So I always color the bad memories with good ones immediately, unless a situation reminds me of it then I don't get angry but I analyze so that again there is no repeat.
