I would think he was buying it for his sister or for a friends birthday...but, If you do think people will think it's weird just buy some wrapping paper with it...

It's similar to a young girl going into a store to buy sanitary products. The purchaser gets self-concious or embarrassed. Fact is, cashiers usually don't even think twice about what people are buying, so it's futile to be embarrased or self-concious about it.

I would think that it was a gift for a his sister,niece, etc. Guys buy dolls all the time. You should've seen them at Christmas. It is normal.

It's just ok...what if the guy has a sister and he wants to give her a present...it's ok...but he should tell the toy store employee that he needs doll for her sister..then tell the employee to carry the doll to the checkout (cashier) then tell them to put it on a bag...(birthday bag)

I think people would assume it was a gift for someone else.
If he's embarrassed, he can askt he cashier for a gift receipt. That way she'll certainly think it's a gift.
