We'll presume that you know how to self-hypnotize. If not, there is an endless wealth of information on the web for process...

You have to reassociate your thoughts and feelings about sugary substances. I would use use a combination of affirmative statements while in state, as well as a visual assocation.

i.e., "I eat food for function, not for flavor. I choose the highest quality nutrients to get the most from my body. I take sugar in moderation and stay aware of my intake." Then perhaps visualize a table filled with sugary items that you enjoy. Now imagine a large red X crossing that image out. Next replace it with a table filled with healthy items.... Fruits replacing junk food for instance, and imagine a large green checkmark running through this image. You might go so far as to imagine eating the junk food and getting an instantly ill reaction.

Above all, remember; it's repitition and conditioning. Don't expect one session to magically rewire you.
