I got the following from Internet Search. However, I used to believe that Pakistan is part of South-Asia, not of Middle-East. Please read the following:

About the second part of your Q., Pakistan has border with Iran and Iran has border with Iraq. It takes around 6-hours flight time from Karachi to Baghdad, depending on the stopover and route you are taking. By road, it may be around 4 to 5000 km (guess only).

It depends on how you look at the Middle East.
The Middle East is traditionally: Egypt, Iran, Iraq, Saudi Arabia, Yemen, Oman, Kuwait, Bahrain, Israel, Lebanon, Jordan, Syria, and the United Arab Emirates.
If you believe that those are the Middle East, then Pakistan is not a part of the Middle East.
However, some people also include: Libya, Algeria, Tunisia, Morocco, Sudan, Somalia, Turkey, Afghanistan, and PAKISTAN
Historically, most of modern day Pakistan was part of the Persian Empire, which was a Muslim empire that also owned most of the modern Middle Eastern territory.
To respond to your second question, Iraq and Pakistan are separated by Iran, which is located between them.
