For example, I have heard that it is customary to remove shoes when entering a house in the midwestern states, but not the New England states. True?

I live in New England and do not allow shoes inside my home. I know several people who have the same rule about shoes in their home. For the most part most people around here just expect that when you visit someones house you will be expected to remove your shoes. Almost everybody I know will automatically take their shoes off in someones home or at least offer to do so. I always take my shoes off it is rude to wear shoes in the house.

I don't know about removing shoes before entering a house, that's probably a personal preference. I know families that have that rule in California, too. I do know I had NEVER seen flip flops in church until I visited family in the south. WOW.

I think it depends on the person/people that own the house. I live in the Midwest, and we allow shoes in the house. But other people that I know do not. Some people are just really picky about keeping their floors clean.

I think it differs from house to house. I know some people who remove their shoes, and I know others who wear them all the time.
