"Ghost codes" are not phone specific but carrier specific. They are virtual numbers that are temporarily issued and used in the background while a number is being ported. To get to the screen where you'd enter this information is on your phone, you'd enter *#639# or *3001#12345# provided your phone doesn't have an MSL on it. If so, you'll have to get that code from your carrier or via software and an FBUS/MBUS cable.
