First off - you can't promise to "never do something again." You can promise to try to never do it again but you can't promise that you will NEVER do it again.
Secondly - Be honest about it. Apologize, Grovel. DO NOT TRY TO DEFEND YOURSELF OR EXCUSE YOUR BEHAVIOR!!!!! This makes the other person MUCH less forgiving. Honesty goes along way and grovelling take it even further ;) Understand that saying you're "sorry" doesn't always fix things. It takes time to prove that you are changing. Be prepared to accept whatever consequences there are. Ask the person to help you do better. And Keep Groveling!

That depends on the level of the offense and are you apologizing to a friend, relative or spouse? (you mention "cheating")
Ask your offendee how you can make restitution. Then fulfil as much of that as is within your power to do so patiently, lovingly and without complaint consistently for as long as it takes and eventually the trust will be regained if you stay on the straight and narrow. Always, the words "I'm sorry" with sincerity and actions to back it up will usually find grace and forgiveness from the hearer.

I would write a heartfelt note stating how sorry your were and that you were only trying to help. Promise that if you are given a second chance, you NEVER do it again and see what happens.
