The cervix must also dialate in order to begin labor. Thinning and dialation usually go hand-in-hand, but it's possible to have some of one without the other. It's not easy to tell when labor will begin. It could be within a few hours, or it could take a few days to even a couple weeks for your SIL to be in labor. These are good signs, though, that labor is going to begin soon and the body is preparing for it. She can try all those "tricks" to get her body to respond and begin labor, like sex, walking, and warm showers, but it's not a guarantee. Her body will begin labor when it's ready, or when a doctor decides to inject her with Pitocen to force labor on her. Have her rest as much as possible to reserve her energy. She will need it to push the baby out!

It really depends, the cervix does not really thin out (or efface) completely until you are nearly all the way dialated. Sometimes labor starts with no effacement or dilation. It does not have to be completely thinned before labor starts; in fact I would think that would be unusual, since it is the labor which *causes* the dilation and effacement.

Your water can break at any time before or during labor, it is not an indicator of anything.

Labor can begin before your cervix has completely thinned. My water broke before I ever started dilating and didn't start dilating until about 12 hrs later. And that was 3 hrs after they gave me meds to thin it. SO unfortunately there really isn't anyway to know.

I also think everyone is different. I was walking around for over a week with my cervix dialated to 4-5cm. But up until this point, I had been told my cervix was paper-thin. So who knows?

Your cervix scaling down is termed Effacement. it is seen the 1st diploma of exertions. that's some hours or few days observing the guy. I even have seen females pass some days until eventually now the exertions actually kicks in. And in my case it grow to be a rely of hours.

If the cervix is already thinning that is part of labor so technically it already has started.
