There are two moments.
1) November 23, 1996 at an ECW house show. Eric Kulas "Mass Transit", conned Paul Heyman in allowing him to wreslte in Axl Rotten's place against Team 3D. Due to a botched blading incident, he was cut severly making the match a way more violent match than before. Due to a video tape of the event, it cost ECW their first PPV event and almost caused them to go out of business.
2) May 23, 1999 at WWF Over the Edge PPV event in which the harness snapped causing Owen Hart to crack his head on the top turnbuckle causing his death.

WCW back in the 90's, I think, The intro of "The Shockmaster", was Tugboat in the WWF. Supposed to be really impressive and it got botched. Had to make his character a bit of a "goof" after that. Was supposed to take on Vader I think.
