Most of the civilized people I know keep the LID up and the SEAT down. If you're a male and want to stand while urinating then you should lift the seat up first to avoid getting it wet. But put the seat back down when finished.
Have you ever woke up in the middle of the night and had to go crap really bad? You rush into the bathroom and sit down only to just about fall into the toilet because the last person to use it left the seat up? When that happens to you, you will learn why you should always return the seat to the "down" position. It's not done just to be courteous to women but to everybody.
Whether you close the lid or not is up to you. I only close it after a shower so I have a place to sit (on a towel) while I dry my feet or between my toes.
If a person is lazy and never cleans the inside of the toilet then maybe they should keep the lid down.

Keep it closed. I do this not only for hygeine reasons, but also to solve the equal rights issue when it comes to men and women sharing a house.
If I'm gonna get **** for leaving the seat up and have to lift it from the female friendly down position, and be expected to return it to that state every time, well, then I'll close the lid just so they have to go through the same motions.
