I've had them from a young age, I remember first getting them at about 13. I'm now 23 and am getting them reguarlly and I haven't had them since 13 which was just the once. For the last 3 years or so I was getting them once a year or so but recently there coming back to haunt me about 2-3 times a month.
My mate told me he had some1 at his work who was off work for about a month when he had them removed. With my job I work a week on week off with only one other person so I know that wouldn't be possible for me to do.

What is the cause of piles?
How can I stop getting them?
Do I need to embarrass myself by bending over and spreading in front of the DOC?

The piles are caused by the veins around the anus collecting blood and becoming swollen. As they swell they can become compressed and irritated. You cannot stop them all together, you can help by not being overweight and not having a high blood pressure.
A doctor will look at them with you laying on your side, he may well use a gloved (lubricated) finger to examine the inside;
Removal is painful post op but well worth it. They may well return though;

Yes you need to see a doctor. Don't worry he will have seen it all before. Piles are enlarged blood vessels around the anus or rectum. It is believed they are caused by a genetic predisposition towards weak blood vessels or by straining during bowel movements. They can be reduced using topical creams (Prep H) or by surgery if they are very bad. This usually involves ligation (cutting off the blood supply) or laser treatment. It's a very quick procedure and the relief is instantaneous. You can prevent haemorrhoids to an extent by drinking plenty of water and eating a high fibre diet. You should see a doctor as untreated haemorroids can cause other bowel complications.

I am not sure but I dont think piles come and go, they are a permanent fixture untill you get them removes, but you dont have to have them removed unless they are something other then piles or the piles are causing you major distress.
You need to emberase yourself by bending over and spreading them in front of the doc, trust me it will put your mind at ease or it will allow you to deal with the issue.

Don't worry about the doc - he/she has seen more a*ses than you've had hot dinners.

When he/she examines you you'll most likely be lying on your side on the examination couch.

Pile removal is pretty painful afterwards. You'd be struggling to get back to work in a week. But if you need the op you have a right to time off (if you're in the UK - don't know about the US).

Your employer would just need to find someone else or make his/her own arrangements.

Your health comes first. See the doc. And take his/her advice on what treatment you need.

You never got rid of you hemorrhoids they just may not have been inflammed at certain periods of your life. They are varicose veins of the rectum and are caused by straining on the toilet and by hard stools. To keep them under control you need to keep your stools soft by getting plenty of fiber and not straining on the toilet. The will be able to check them with you lying on the exam table.
