Italy is a big place. If you have only a short time, spend it all in Rome and nearby Pompeii.
Venice was the only place that is as beautiful as the pictures.
In Rome, of course the Colosseum etc. are "must-sees."
But the Basilica of San Clemente is also a fantastic, often-missed site. The ground floor is a 12th century church still in use (I believe). Go down a floor and it's a 4th (I think) century church. Down another floor is a Roman street and a Mythric temple.
In Pompeii: DO NOT go on one of those horrible bus tours. The trains, like the blood runs on time in Italy. Take a train and go there yourself. Don't forget all the treasures are housed in a museum now (not on site). It is amazing.
The main plaza in Florence is also gorgeous.
I didn't do it, but culinary tours/classes are very popular. You spend time with a chef, going to real Italian street markets, preparing real Italian food. Sounds yummy.

P.S. I don't believe Pomeii is an island. Wikitravel.org is a good source for basic info.

I would say Rome, Florence, and Venice however I would put Naples (Pompei) as a definate not miss as well. I would put Pisa as a miss - except for the famous tower there isn't that much to do there. The Isle of Capri is also nice to visit.
