a thorastic surgeon or neurosurgeon or psychiatrist in a high income area.
any specialty that allows few new people in is usually high paid.
The sad thing is the more patients with expensive problems you cram into your schedule a day the more money you receive.
I try to avoid such doctors.

Probably neurosurgery. It's a risky procedure and only the best of the best get to practice it.
