I don't know your price range, but how about:
A crutch or leg cast/brace with a note saying, "Break a Leg!"
A piece of jewelry with the theatre masks or something from the show (a charm of running shoes for "See How They Run", or something like this: http://www.shakespearesden.com/theatrejewelry.html)
A directors chair with your friend's name embroidered on it
A framed photo or Playbill from the original production of the show, or get the current cast to sign a program and frame that for her
Some accessory to go with her costume
Basket of back-stage snacks: waterbottle, nuts, oranges, gum, chocolates, popcorn, etc.

The best gift is a memory of the show.
If you frame the program or poster, have a photo from rehearsal or a group shot of the cast ~ these would all be cherished for years to come.

A Moby Wrap. It's not something I would have bought for myself, but when my sister (who's a doc) got it for me, I knew I had to learn how to use it. If the baby is born in the summer, it might be a little hot to use it, but it is the most comfortable of all wraps, easy to use and we (DD and I) love it. Especially when she was a newborn. Everyone also comments on how cool it looks. Or a swaddle blanket.

I always think something like a drama pin or a small music box with a favorite song is something unexpected as well as nice. Nothing fancy but something unique.

What better gift than a book (relating to the play or a just fun or amazing book) or something vital to actresses? Maybe a handmade t-shirt with her picture in her role.
