She is just seeing the ghost of her 'friend'. It really has no meaning to it other than to let you know that her friend has just died because of the crawlers in the caves. The point of the movie, don't go down into deep dark holes even with people you trust.....

The movie is just a tiny piece of a really good story you should read the book it's awesome. I didn't really like the movie because I had already read the book but you should check it out.

The point is that there is no hope...everyone dies. Thats what makes this movie stand out from the other crappy hollywood endings where someone always comes out ok.
The ending is purposefully bleak. The blond chick goes mad and accepts her fate as she sees her dead child...soon to join her.
The further they descend the crazier they become.
Good movie.

The way I saw it: Sara decides to stay down there and become a crawler as well. She has nothing to go back to (child and husband dead, husband cheated, friends are all dead). So she'll become one of the crawlers. She had that crazy look in her face when she slashed Juno in the end...
So that's what I'm thinking.
I really liked the movie, didn't expect to. But it turned out real well.
