Your doctor's word is final. If he says you are fit for work (doctor code for "I've got a right malingerer here") then you are fit for work.
If you disagree, then you must change your doctor, which is easier said than done and in any event, gives you no guarantee of a change of opinion.
Neither nervous anxiety (whatever that is) nor rheumatoid arthritis should prevent you working, at least part time.

Change your Doctor to start with - if he has been giving you certificates up to this point and then suddenly decides that you are fit for work then you may have to consider the fact that he may be being investigated - but that is rare!
If you are under a consultant - it may be him that is causing the problem - he may have informed the doctor that in his opinion you are fit to work!
The other thing to consider is the fact that you are legally entitled to see what is on your medical record at your Doctors - he/she cannot refuse - that also may give you a clue to his conclusions!
Make an appointment to see you doctor and re-iterate your points to him! Not many employers would take you on whilst on medication for anxiety anyway! Unfortunately, in some circumstances, you have to stand up to the doctor and be strong!

May be you are fit for work. But after 12 years you have lost confidence in your ability to do so. Why don't you try to get back into some kind of paid employment? If that doesn't work out your doctor may feel more inclined to sign you a sick note.

Firstly you are entitled to see whatever doctor you wish to. I would see another doctor and see what he/she has to say.
If you have been out of work for 12 years (I presume on income support) then why is your doctor now saying that he won't sign a form for you? Is it a different doctor you are seeing?
A suggestion, see your consultant and ask if he/she will sign your note that you need. If you have been under them for a while, then you shouldn't have any problems.

see another doctor...i went thro this...i was clearly not fit for work but my (alcoholic) doctor refused to sign me off sick...you can make a claim...it would be alot easier if you had the support of your doctor however...have you told your consultant all this....good luck and hope you feel better soon...it is your right to have a second (or 3rd) opinion.....

i know how you feel i have panick attacks and also tendonitus and bursitus in my right shoulder i know the pain you go through believe me i do . I live in canada and had to go on welfare a couple of years ago. because i cannot work anymore i have no mobility in my right arm at all
. the welfare wanted me to go to work or they were going to cut my cheque. So i went to my family doctor he put me on antidepressants, and said i could bnot work for a while he doeasnt know for how long so far. so i got a little bit more money from welfare every month for one year but after that i have to go see them again
