Open Door Policy means that your boss is open for any discussions, issues, or concerns to their attention at any point in time. They are there for you to listen, mentor, and guide you.

it means her door is open, as in anyone can come to her to discuss anything about any aspect of the job, any problem with co-worker, etc. Supposedly she willl take the info, and do something if necessary without naming you as the source, or take steps to fix any problems, or implement any good suggestions. It doesn't mean she wants to hear personal problems.

"Open Door Policy" means that you can go to her at any time. You don't have to be afraid to approach her with issues, problems, etc.
