Well first of all you should make sure you have a properly fitting belt and retighten it when you are at depth to help avoid the problem.
If it does come undone underwater because you are swimming in a horizontal position and it is buckled in the front of your torso it shouldn't fall off but begin to slip, at which time you need to grab it and retighten or at least hold onto it and safely ascend to the surface.
If it comes clear you immediately need to dump any air you may have in your BCV and assuming you are close to the bottom swim down as fast as you can to grab the weight belt, rocks or anything you can to help you stay on the bottom while signalling for another diver to come and help you retrieve the weight belt.
The final scenerio if you cannot reach the weight belt, bottom or have anyone to assist you is too first dump all the air in your BCV, flail out in the star fish shape to slow ascent as much as possible exhaling all the way to the surface to help prevent burst lung syndrome. Once at the surface inflate your BCV and signal to anyone around that you need assistance immediately, unless necessary do not overly exert yourself stay as calm and breathe as easily as possible in the situation.
Once out of the water go on 100% O2 as soon as possible and have someone transport you to a hospital, preferably one with a hyperbaric chamber. This is just a precaution, you probably wont need it but it is better to be there in case you do.
The last scenerio is the worst case and has very little chance of ever happening as long as the diver follows the simple procedures and pays attention to their equipment both before and during a dive.

Grab your weight belt with your and and pin it to your hip and roll into it and re-tighten it. I would practice this so you know what to do if it actually happens

you deffenitly need to control your accent and empty your bouyancy regulator.
