Anything around the 16" center line of the drain should do the trick. You have enough adjustment in your pipe to allow if you're off some one way or the other.

All depends on what type of cabinet and sink, or pedestal you want to install. If your plumbing is vented in the wall through the roof a typical cabinet and drop in sink drain stub out is usually in a good spot at about 16" to 20" off the floor. Pedestals you generally want an inch or two higher because of the design of the leg. some are solid except just at the top third. if its in a cabinet you are safe if you are at or below the end of the sinks tailpiece by a little. You can always add extentions to bring the drain down. You cant however make the water go back uphill unless you want to add a pump, which in most cases is just plain rediculas. If there is no vent stack and you had a funny looking contraption under the sink ( p- trap is connected to a sanitee, on top of the sanitee is a studor vent) you will want to be within a few inches of the bottom of the cabinet to be able to rise up and use the studor vent again. Also be aware of drawers that might get in the way!

Depends. The toilette drain is straight down, unless its a wall hung then the manufacturer tells you in their literature.

The tub and/or shower drains are straight down through the floor.

The vanity drain usually goes into the wall at about 16" on center. It might be wise to offset some from the center line of the vanity drain to allow for extra wiggle room which is sometimes (almost always) nice. I try to get at least 2" and not more than the length of the trap.
