All the Silent Hills are good and scary, but I think Silent Hill 4: The Room is the creepiest.
Resident Evil 4 is not so much scary as suspenseful and edge-of-your-seat thrilling. Check out any game site and read the reviews. This is considered to be an almost perfect game. Now that's it's out for PS2, you should give it a try.
I think, along with Silent Hill 4, the creepiest game I've ever played was Siren. The plot is very convoluted, and it can be hard to follow, but this one really raised the hair on the back of my neck, and there are moments of suspense that will put a knot in your stomach. The game play is similar to Silent Hill, meaning you don't have an arsenal of great weapons (sometimes your forced to use a piece of wood), and your character(s) aren't that strong or fast. You have to be really patient to play this, as you'll be going back to attempt levels over and over again. It's not perfect, there are flaws in the design of this game, but you wanted creepy and this is it.
I also agree with Slazinger, Fatal Frame is a great, scary game.

Ive played all of the silent hill series and i reckon they are very good. I consider them to be more disturbing than scary though..
Project zero 3- Fatal Frame, i strongly reccommend it if you are looking to sleep with your lights on for a couple of weeks.Very scary indeed- a must for fans of japanese horrors.

You ruined the question by already doing Silent Hill... that is THE scariest I can think of.
F.E.A.R is supposed to be scary but I haven't played it...

Shadow the Hedgehog. the game is so bad it'll scare you.
Devil May Cry isn't necessarily scary but you kill a lot of things.

Code Veronica...that one really did give me nightmares (the only game ever). Probably a bit dated now, but amazing at the time.
