It really depends on the provider.
Usually when they say specs, they will only tell you the download speed. Like 256KBps in your example. It is roughly 28Kilobytes per second.
Unfortunately, they can set the upload whatever they like although generally it's either the same or half than the download speed, so it is best to ask them that is the download speed AND upload speed of a speed package.

That is determined by your provider. DSL typically has slower upload speeds than download (about half). Cable is very close to being the same speed both up and down.
Call your company and ask.
