Go to their teams website and look up the stadium adress and mail a letter to the team c/o the player. For example:

The Atlanta Braves
c/o Chipper Jones
Turner Field
755 Hank Aaron Drive
Atlanta, GA 30315

Make sure to send a sase and a protector. Chipper is one that I actually had sucess with. He sent back a signed 5x7 photo.

Beckett magazine publishes a book of addresses of players homes and PO Boxes. You could also send them to their home stadiums. Those addresses are on mlb.com. Be prepared to wait though, and don't send valuable cards, you may never see them again. All in all, you have about a 50% success ratio.
