This expression came about because immigrants to the Americas talked about the "Old World" versus the "New World." But the world that was neither became known as the "Third World."

China, Soviet Russia, and Cuba. It is a term applied to communist nations. Fourth world (if you wanted to know) includes ghettos, where there is absolutely no contact with the rest of the world.

The second world is canada and iceland and sweden and finland and russia and china and indonesia and australia and so forth. civilized but still old fashioned.
USA and japan and england are all first world.
