When? 13.7 billion years ago. How? Well think about it like this: There was empty space forever and ever. Sometimes empty space has flucuations in energy. So a photon of light will appear one place and an antiphoton somewhere else. Entirely random, and in net there is no energy. But randomly this zero energy state will split into a positive and negative and then annihilate back to zero. Well this was going on forever until one day, randomly, a bunch of photons appeared in the same vicinity and voila! Energy turns into mass, and mass explodes into outerspace. (E = mc^2) Still today, the mass continues to travel outward in what we call 'expansion' of the universe. Beyond the mass, lies the original open space.

I believe answer to this question lies in Indian(Hindu) mythology.I haven't found it yet,but someday somebody will.

Some 20 billion years ago there was only 'nothing', a 'nothing, about which we do know nothing. It was literally 'nothing', because nothing which we know today ( name anything you like- sky, space, gravity etc) existed. Some called it 'God' for want of a better term. Because, we assume that it always existed, it is & it will. Suddenly, it started 'becoming' all that we know today.
