There are many areas in the United States that have no high speed connectivity. I'm not sure if they include satellite but satellite is not really competitive with true broadband. Most are rural and not near any major metros. Basically, the broadband providers, DSL/cable/wifi, do a market analysis on a region to include population density, average age and income levels to project how many customers in the area will be subscribers. They use these figures as well as the estimated cost of the infrastructure to determine at what point they'll get their money back for the installation. Generally, most companies use 18 months as a term. If the are doesn't have the population statistics they providers won't install. according to gartner around 42% of all the people in the US have high speed (IE broadband) internet connections. The top reasons for not having is lack of availability, to pricey, and finally not internet users.
In that order.

I dunno about BT, but I've heard the same BS here in America.

They say it based on population and usually they add in the people who COULD get it if they wanted to (like in cities) but don't - cause they have it at work - or don't have a computer.

Compared it that way to the one's who can't (and I don't know how they figures that exactly) - the percentage ends up grossly overstated.

So, take 100% of the population (including children?) in every city or area that COULD get it - compared to the places they care to look for people who can't - is the only way it adds up for me.

ISTR that this is 99% of the population NOT 99% of the area.

If you want something faster and cant get xDSL, you may want to look into ISDN x2 it does cost a bit more, but at least you will get faster speeds in a only dialup area.
