(45) PNEUMONOÂULTRAÂMICROÂSCOPICÂSILICOÂVOLCANOÂCONIOSIS (also spelled PNEUMONOÂULTRAÂMICROÂSCOPICÂSILICOÂVOLCANOÂKONIOSIS) = a lung disease caused by breathing in particles of siliceous volcanic dust.
This is the longest word in any English dictionary. However, it was coined by Everett Smith, the President of The National Puzzlers' League, in 1935 purely for the purpose of inventing a new "longest word". The Oxford English Dictionary described the word as factitious. Nevertheless it also appears in the Webster's, Random House, and Chambers dictionaries.

there is a 1,913 long word that is used to describe a synthesised protien. it was published in a medical journal, so is the longest word in existence, but is not technically a 'real' word as has no usefull addition to civilised conversation, unless of course you are a scientist and had 3 hours spare to learn the full name of it.

Even if we limit the discussion to one language, I don't believe there is one correct answer. This because languages are flexible and fluid. I remember studying biology and learning some fantastically long terms, but these terms can be combined to describe other processes or conditions. The combination becomes a longer word that can then be combined with others to describe even bigger processes or conditons.
I could then make them even longer by applying a suffix or prefix.
Language is simply to flexible for only one answer to this question. Sorry.
Regards
