The most difficult thing? To watch a loved one die in pain because of a sickness. My grandmother passed away last year of diabetes/heart disease and it was very difficult watching her suffer

I would say the loss of a child. In a less emotional arena I'd say it is walking the line of righteousness before our Father in Heaven. Controlling ones tongue is no easy feat either.
