In my experience, it hurts just as much giving the head butt, but i always pretend it doesn't hurt since i'm the one that gave it.
Headbutting the wrong place can hurt you more than them, and also cause concussions
If the reciever does feel more pain, its probably due to shock, for instance if the headbutt is unexpected.

You have hit it on the head what is wrong with this head butt thing. It hurts the giver just as much as the receiver.

Depending on where you are butting the person leads to whether or not it hurts.
skull to skull hurts both
skull to nose hurts receiver
Unless you have really spiky hair... no real contact
