I think that people choose to be anorexic for themselves, until there is a point it becomes too difficult to quit. The fashion industry and media has something to do with it, but there is much much more to it, I would think.

YES - they demand that the girls be skinny - look at the clothes they model. A "normal" woman could never fit into anything they wear.

Other. While they are part of the problem, "we" the consumers are the cause of it. Incredibly skinny models wouldn't have a job if "we" the consumers didn't buy it.
