I want to express my sincere appreciation to my boss for his kindness and trust for giving my an excellent evaluation that will reflect on my salary. This is the third nice increase I got from him. Can you help, please.

What does he like?

tickets to a ball game?
memorabilia of some sort?
does he collect something?

put some thought into it...those are the gifts people appreciate the most

if you don't want to buy him something, maybe just work your *** off to show him you're worthy of it

and congratulations

I agree with most of the other answers, a sincere thank you and letting him know that the gesture is much appreciated will be enough. Don't buy him anything! Seriously, I don't know what people are thinking. THis is business. He's not giving you his own money, you did a good job and deserved a raise. Thank him for acknowledging that but don't start to look like you're buying your raises and make him uncomfortable. It's professional, not personal. Congratulations.

First, tell us where you work! haha
A simple heartfelt thank you with a statement or two about how much you like working for him on a good stock card would be great. (in your handwriting, not computer printed). Be careful of the wording if you and the boss are opposite sexes though.
People tend to go for gifts and gadgets, but in today's times, having a great card to keep and look back on during the not so great times can be a huge boost to people's spirits.

Write him a quick letter expressing your appreciation for the raise and that you are happy that he felt you deserved it. Depending on your job, after you give it to him, you could also ask him if there is anything you can do to take some work of his shoulders.

A "hey thanks for the good review."
You said it will reflect on your salary
which means that he is not the one to make the decision to give you the raise.
The raise you are getting is A thank-you from the company for either working hard, or an annual increase.

first you could tell him. second a nice thank you card would be in order. i'm not sure i would actually buy him something. but since it's the third increase, you might find something he has as a hobby and get him something...if he like's nascar, get him a metal minature. if he likes golf, get him a round at a local club or invite him out one day for golf and lunch.

A nicely written thank you note will do.

If you buy something or give him something, it will look like you are trying to buy his favor. And you'll come off looking like a kiss up.

Enjoy the raise and thank him by working even harder!!

As hard as it is, don't thank him for the salary increase. You earned it, didn't you? You only have yourself to thank, and if you let him think he's wholly responsible for your success, eventually you'll both come to believe it . Continuing to perform at the high level he expects from you is thanks enough.
